Introduction
At Concise Healthcare, LLC, we are committed to the protection and confidentiality of the health information entrusted to us. This HIPAA Policy outlines our practices in relation to the handling of Protected Health Information (PHI) in compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the subsequent amendments including the Health Information Technology for Economic and Clinical Health (HITECH) Act.

Our practice is dedicated to maintaining the privacy of your PHI and it is required by law to maintain the confidentiality of information that identifies you. This HIPAA Policy describes how we may use and disclose your PHI to carry out treatment, payment, or healthcare operations and for other purposes permitted or required by law.
Use and Disclosure of PHI
Concise Healthcare, LLC will not use or disclose your PHI without your written authorization, except as described in this policy. We will limit the use or disclosure of PHI to the minimum necessary to accomplish the intended purpose.
Treatment
We will use and disclose your PHI to provide, coordinate, or manage your healthcare and any related services. This includes the coordination or management of your healthcare with a third party, consultation with other healthcare providers, or referral to another provider for health care.
Payment
Your PHI will be used, as needed, to obtain payment for the services we provide. This may include certain activities that your health insurance plan may undertake before it approves or pays for the healthcare services we recommend for you.
Healthcare Operations
We may use or disclose, as needed, your PHI to support the business activities of our practice. These activities include, but are not limited to, quality assessment, employee review, training of medical students, licensing, and conducting or arranging for other business activities.
Other Permitted and Required Uses and Disclosures
Other uses and disclosures of your PHI will be made only with your written authorization, unless otherwise permitted or required by law. You may revoke this authorization at any time, in writing, except to the extent that your healthcare provider or the practice has taken an action in reliance on the use or disclosure indicated in the authorization.
Your Rights
The following is a statement of your rights with respect to your PHI and a brief description of how you may exercise these rights.
- Right to Inspect and Copy: You have the right to inspect and copy PHI that may be used to make decisions about your care.
- Right to Amend: If you feel that the PHI we have is incorrect or incomplete, you may ask us to amend the information.
- Right to an Accounting of Disclosures: You have the right to request an accounting of certain disclosures of your PHI.
- Right to Request Restrictions: You have the right to request a restriction or limitation on the PHI we use or disclose for treatment, payment, or healthcare operations.
- Right to Request Confidential Communications: You have the right to request that we communicate with you about medical matters in a certain way or at a certain location.
Changes to This Policy
We reserve the right to change this policy and make the new policy effective for all PHI we maintain. The current policy will be posted in our office and on our website.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services.
